Job Title: Clinical Primary Therapist Profession: Social Worker Specialty: Acute Social Work Duration: 26-week contract Shift: Full-time, Days Hours per Shift: Monday to Friday, between 8 am and 7 pm, with on-call rotations for off-shift, weekends, and holidays Experience: Minimum of 2 years as a LCSW or LMSW required License: LMSW, LCSW, LMHC, or LCAT required Certifications: BLS, Covid and Flu Vaccine required Description: Master's Degree in a relevant field is required. Candidates must be familiar with state requirements and should have prior work history in the relevant jurisdiction. Experience working with clients aged 5 and older is essential. This position involves providing crisis intervention and counseling services. The Clinical Primary Therapist works with clients both individually and in group settings to address mental health concerns. Knowledge and skills are applied to assist clients in recovery and illness management. The Therapist is responsible for establishing the boundaries of therapy and creating a safe environment for clients and families. A comprehensive assessment is conducted to gather data for developing a treatment plan. Collaboration with clients and their families is crucial to establish treatment goals and identify interventions for goal achievement. The Therapist must ensure timely and accurate documentation in the client's medical record. Coordination of care with colleagues is essential to secure appropriate treatment resources. Participation in on-call rotations to provide mental health crisis evaluations is required. Comprehensive assessments include psychosocial history and evaluations of lethality, substance use, and overall health status. Diagnosis and potential problem statements must be derived from assessment data. Treatment plans are person-centered, recovery-oriented, and culturally responsive. Goals and objectives in treatment plans should be clearly linked to assessments and discharge criteria. Progress notes and documentation must accurately reflect services provided and client progress. Evidence-based interventions should reinforce healthy behaviors and social integration. Quality and effectiveness of therapy practices need to be systematically evaluated. Collaboration with clinic team members to achieve productivity standards is essential. Self-evaluation of practice is encouraged to identify strengths and areas for professional development.
