Job Title: Social Worker Profession: Social Worker Specialty: Clinical Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 hours Experience: Minimum 1 year of hospital case management (discharge planning) experience required License: Active social worker license Certifications: LICSW required Must-Have: - Hospital case management experience - Knowledge of Cerner Description: Fulfills the needs of patients, families, and the community by providing social work services in accordance with licensure, certification, training, and regulatory requirements. Assumes the responsibility for coordinating and facilitating services for patients during and subsequent to admission to the acute rehabilitation unit. Conducts discharge planning, coordination of care, and other planning and counseling activities to effectively move the patient through the continuum of care in a cost-effective and resource efficient manner. Responds effectively to new and changing work assignments.
