Job Title: Social Worker II Profession: Social Worker Specialty: - Duration: 13 weeks Shift: Days Hours per Shift: 8 Experience: Hospital experience preferred License: LMSW or LCSW Certifications: - Must-Have: Must have Oklahoma license at time of submission Description: Under general supervision, provide social services to patients and their families. Responsibilities listed are core to the position. Inability to perform these responsibilities with or without an accommodation may result in disqualification from the position. Provide social services to patients and their families for an assigned medical service. Conduct discharge planning evaluations of patients and/or family members to gather needed information. Make psychosocial assessments and relate findings to necessary hospital staff. Counsel patients and other persons regarding diseases, treatments, social or financial situations, emotional stress, and other problems. Conduct high social risk case findings and provide crisis counseling to patients and families regarding child abuse or neglect and physical abuse, documenting these sessions. Provide services of crisis intervention for patients and families. Act as a liaison between patients/families and hospital staff and other agencies. Utilize available information to evaluate the effectiveness of services provided in producing the desired outcomes. Make adjustments in patient care as indicated. Assist in discharge planning and referrals to the community for ongoing services. Assist patients and families with post-hospital placement or transfer to post-acute care facilities. Assist patients and families with adverse reactions or difficult adjustments to illness, treatment, or changes in functional status. Utilize specialized interviewing and communication skills with children. Act as a patient advocate for issues related to informed decision-making, end-of-life care, realistic goal setting, and appropriate resolution of ethical dilemmas. May prepare statistics concerning referrals, extended stays, and other elements. May supervise students or newly hired employees. Perform other duties as assigned.
