Job Description:

The Hospice teams support patients and family members through the final stages of their illness, offering services in the home setting, including skilled nursing visits, pain and symptom management, medical social services, and bereavement support.

Responsibilities:

• Provide hospice care and support services to patients and families in various settings.
• Coordinate care and resources with skilled nursing facilities, assisted living facilities, and adult foster homes.

Requirements:

• Oregon LMSW or CSWA licensure required.
• Healthcare Provider Level BLS required.
• Must use personal automobile; valid driver's license and insurance required.
• Oregon LCSW preferred.
• Experience as a social worker in a hospital or medical setting preferred.
