Job Title: Social Worker Profession: Social Worker Specialty: Medical Duration: 16 Shift: Regular Shift Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum Years of Experience required License: Bachelor’s degree in social work is required Certifications: None specified Must-Have: None specified Description: This position will be based at a health department and cover multiple sites. The Clinical Care Team will take referrals from primary care providers. The team will work with the primary care team to accomplish various tasks related to social support. Responsibilities include social support navigation for social determinants of health such as food and housing insecurity. Duties entail compiling and maintaining a resource list for SDOH resources, including eligibility criteria and referral processes. Collaboration with primary care nurses and providers is essential. In-person or remote social needs screening and assessment for primary care patients referred by a nurse or provider will be conducted. Coordinating or making patients aware of social service resources such as housing, clothing, food, and mental health services is required. Collaboration with other social workers to identify patient and community resources is necessary. Conducting case management activities is also part of the role. This includes working with hospitals for discharge planning, follow-up, and education. Assisting in obtaining patient records from hospitals is expected. Assisting in securing needed medical equipment through community partners will be part of the responsibilities. Conducting follow-ups on care plans is essential. Identifying patients lost to follow-up or overdue for care and assisting them in returning to care is required. There may be assistance needed with specialty referral navigation. Tasks may include scheduling, coordinating, and tracking non-specialist and imaging referrals. Assisting in obtaining patient records from specialists and imaging centers is necessary. Compiling and maintaining a resource list for specialty referrals including eligibility criteria and referral processes is part of the duties. Assistance to patients in locating and accessing low-cost prescription options like patient assistance programs and discount retailers will be provided. There may be assistance with patient assistance program applications and serving as a patient-provider liaison with drug companies. Helping patients with applications for programs like CoverRx and RxOutreach may also be required. Other regional primary care-based initiatives with a social work component may be part of the responsibilities. Documenting in patient records, updating consults, and tagging providers or clinical staff as necessary is expected. Providing patient education or finding appropriate educational resources will be part of the role. Expectations may include completing onboarding and orientation. Participating in regional office and primary care clinical meetings as requested is necessary. Attendance at provider meetings may be requested. Throw an effort to attend health councils and other community meetings to build relationships with social service agencies is encouraged. Identifying barriers to care or assistance experienced by patients and seeking ways to address them is an essential duty. Tools and Equipment include personal computers, telephones, fax machines, printers, scanners, copy machines, calculators, personal vehicles, and other office-related equipment as required.
