Job Title: Medical Social Worker Profession: Medical Social Worker Specialty: Hospital Duration: 8 weeks Shift: Full-Time, Days Hours per Shift: 8:00 AM - 4:30 PM, Weekdays and Weekends Experience: Acute care experience required License: ASW or higher license required; LCSW preferred Must-Have: - Strong psychosocial assessment skills - Strong critical thinking skills with independent judgment - Skilled in working with patients and families coping with loss and grief - Excellent follow-through and organizational skills with attention to detail - Basic proficiency with word processing and databases - Strong written and oral communication skills - Ability to work effectively with diverse populations Description: Provides direct clinical social work service, counseling, discharge planning, and case management services to patients and their families. Assesses and documents emotional, social, financial, environmental, or follow-up referral needs. Works collaboratively with the interdisciplinary healthcare team. Formulates a psychosocial plan of care related to the assessment of patients' unique strengths and needs. Develops a discharge plan based on the care team's designations. Monitors progress toward care goals and identifies barriers to meeting estimated length of stay. Facilitates integration with community-based resources to optimize patient health. Activities are consistent with the organization's mission and values and compliant with applicable regulations and policies. Partners with physicians, nursing, and other healthcare team members to establish, document, implement, and evaluate a plan of care. Brings expertise in psychological assessment, acute and long-term care, and community health to the interdisciplinary team. Monitors the progress of the plan of care and communicates any changes with the care team. Provides integrated mind, body, spirit service to patients and families through a mutually agreed-upon plan of care and advocate relationship. Supports processes related to health management of patients across the continuum of care. Assesses needs of patients and families and conceptualizes problem situations within social work scope of service. Provides direct social work services and counseling to patients and families. Interprets social, psychological, emotional, financial, and family issues for attending physicians and other health care team members. Completes required documentation in patients' medical records and relevant systems. Demonstrates continued enhancement of professional skills. Complies with department and hospital policies and procedures. Supports organizational goals to provide optimal care to patients, families, and community. Performs other duties as assigned.
