Schedule: Flexible 8-hour and 12-hour shifts (Day, Evening, Night)

Position Overview

"Navitas Healthcare, LLC" is seeking a compassionate and reliable Travel Licensed Practical Nurse (LPN) to join a Skilled Nursing Facility (SNF) in Hancock, MI. In this role, you will provide high-quality, patient-centered care while supporting residents’ comfort, safety, and overall well-being. This opportunity is ideal for LPNs who excel in fast-paced environments and are committed to delivering exceptional care.

Key Responsibilities

• Deliver direct patient care in accordance with individualized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor patient conditions and report any changes to the RN or physician promptly
• Assist residents with activities of daily living (ADLs), including hygiene, mobility, and feeding
• Maintain accurate and timely patient records in compliance with facility standards
• Adhere to infection control protocols and safety guidelines
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes

Required Qualifications

• Active LPN license (state-specific or compact)
• 1–2 years of experience in a Skilled Nursing Facility (SNF) or similar setting
• Strong clinical skills and a patient-focused approach
• Ability to adapt quickly in a travel assignment environment

Education

• Diploma or Certificate in Practical Nursing from an accredited program

Licensure / Certification:

• AHA BLS Certification
• COVID-19 Vaccination Card
• Driving License

Preferred Experience

• Experience in long-term care, rehabilitation, or SNF settings
• Familiarity with Electronic Medical Records (EMR) systems
