Position Summary
The Licensed Clinical Social Worker (LCSW I) provides clinical social work services to underserved individuals with complex medical and psychosocial issues, focusing on reducing distress and improving mental health. The role works as part of a multidisciplinary team to deliver assessment, psychotherapy, crisis intervention, care planning, and coordination across community and medical settings.

Key Responsibilities

  • Provide comprehensive psychosocial assessments, diagnoses, and individualized care/treatment plans for adults with mild to moderate mental health disorders and co-occurring medical or social problems.

  • Deliver individual and group psychotherapies and/or psychosocial rehabilitation for conditions such as anxiety, depression, chronic pain, substance use, personality disorders, and challenging behaviors.

  • Offer short-term and crisis counseling to stabilize patients and support ongoing engagement in care.

  • Serve as liaison between the clinic and community agencies to integrate services, coordinate care, and support relationship-building and public relations.

  • Collaborate closely with medical providers, nurses, social workers, case managers, and front office staff as part of a multidisciplinary team.

  • Refer patients to community resources, specialty mental health, psychiatry, support services, and institutions as clinically indicated.

  • Provide consultation to team members regarding the biopsychosocial implications of illness and inform medical treatment planning.

  • Conduct functional assessments and prepare reports for entities such as DPSS, DCFS, Social Security Administration, and courts.

  • Provide limited case management or referrals to assist homeless or vulnerable patients with housing, public benefits, substance use treatment, and other vital resources.

  • Document all assessments, diagnoses, treatment plans, interventions, and referrals in the electronic medical record in a timely and accurate manner.

  • Maintain confidentiality and comply with all clinic policies and professional standards.

  • Demonstrate supportive, client-centered engagement that respects individuality and cultural differences.

  • Function as a team player with strong communication, collaboration, multitasking, and independent work skills.

  • Perform additional duties and special projects as assigned.

Minimum Qualifications

  • Active California license as a Clinical Social Worker (LCSW).

  • Minimum two years of experience providing mental health services (assessment, diagnosis, individual and group therapy) to underserved, homeless, mentally ill, and/or substance-addicted adult populations.

  • Prior experience in a community mental health or primary care/medical setting preferred.

  • Current CPR certification.

  • Proficiency with computers and electronic medical records.

  • Bilingual Spanish/English preferred.

Schedule

  • Full-time, 8-hour shifts.
