Pay Rate: $89,273.00 to $116,055.68/yearly.

Summary: The Clinical Social Worker provides psychosocial assessment, counseling, care coordination, and discharge planning services to patients and families in an acute care medical setting. This role supports patients coping with illness, injury, and hospitalization by addressing emotional, social, environmental, and financial factors that impact care and recovery. The Social Worker collaborates closely with interdisciplinary healthcare teams to ensure safe, timely, and appropriate transitions of care.

Responsibilities:

• Conduct comprehensive psychosocial assessments of patients and families.
• Provide short-term counseling, crisis intervention, and emotional support.
• Assist patients in coping with illness, trauma, disability, and end-of-life issues.
• Identify social determinants of health affecting treatment and recovery.
