Summary: The Acute Care Social Worker (Medical) – Discharge Planning supports hospitalized patients and their families by coordinating safe, timely, and appropriate discharges. This role focuses on psychosocial assessments, resource coordination, and collaboration with the interdisciplinary care team to ensure continuity of care post-discharge.

Responsibilities:

• Conduct psychosocial assessments for hospitalized patients.
• Develop and implement discharge plans in collaboration with the care team.
