Job Title: Licensed Clinical Social Worker Profession: Social Services Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 1 year in inpatient/acute care, case management License: LCSW Certifications: BLS, LSWAIC Must-Have: - Epic experience - Ability to float to other facilities if necessary - Minimum of 2 weekend days per month Description: Candidates who have been separated from another facility for at least 3 months are eligible for this position. First-time travelers will be accepted. The clinician will engage in care coordination for neuro and cardiac patients. This includes working in a comprehensive stroke center. Prior hospital experience is required. The average patient ratio during the day is 20 patients. The selected candidate will participate in care conference activities. Responsibilities include assessing biopsychosocial needs, developing plans of care, and coordinating services for discharge. The role requires documentation according to the established plan of care. Crisis intervention skills are essential. The candidate will refer patients to various resources such as rehab and long-term care facilities. Knowledge in areas such as advanced directives, medication assistance, and community resources is beneficial. The clinician will collaborate with interdisciplinary support including therapy and rapid response teams. Additional duties include conducting needs assessments and ordering durable medical equipment. Understanding and familiarity with social services resources is important for success in this role.
