This role includes providing assessments, facilitating groups, supporting treatment planning, and offering crisis intervention. Conduct client assessments and develop individualized treatment plans. Facilitate group and family sessions. Document client progress and update care plans. Provide crisis intervention and aftercare planning. Collaborate with referral sources, insurance, and care teams. Deliver education to clients, families, and the community.
