State or federal-issued photo ID must be presented at the facility and worn as a photo badge at all times.
Responsibilities
- Implement OT treatment plans developed by the supervising Occupational Therapist to restore and enhance residents' functional independence.
- Deliver skilled OT interventions including ADL training, fine motor exercises, cognitive rehabilitation, and adaptive equipment instruction.
- Document patient progress accurately and in a timely manner in compliance with facility and regulatory standards.
- Collaborate with the interdisciplinary team OT, PT, SLP, nursing to support integrated, resident-centred care planning.
- Educate residents and families on home exercise programmes, compensatory strategies, and safe discharge planning.
- Meet productivity standards while maintaining the highest quality of care.
Requirements
- Current, unencumbered COTA licence/certification in New Mexico required.
- Active NM Caregivers Registry account required prior to assignment start.
- Graduate of an ACOTE-accredited occupational therapy assistant programme.
- Minimum 1 year of clinical experience in a SNF, transitional care, or post-acute rehabilitation setting preferred.
- Current BLS/CPR certification.
- Strong documentation skills and ability to meet productivity targets.
