Transportation Coordinator

Position Summary

The Transportation Aide is responsible for coordinating, scheduling, and providing safe transportation for residents to and from appointments, while ensuring their comfort, safety, and dignity. This role also includes chaperoning residents during appointments as needed and supporting overall care needs in alignment with Topside Manor’s standards.

Key Responsibilities

• Coordinate and schedule resident transportation for medical and other necessary appointments
• Safely transport residents to and from appointments in accordance with facility policies
• Accompany (chaperone) residents during appointments when required
• Communicate effectively with nursing staff, families, and healthcare providers regarding scheduling and resident needs
• Ensure timely arrival for all scheduled appointments
• Assist residents with transfers, mobility, and personal needs during transport
• Maintain transportation logs, schedules, and required documentation
• Ensure vehicle cleanliness, safety, and routine maintenance checks
• Follow all safety, infection control, and HIPAA guidelines
• Participate in holiday rotation and assist with coverage as needed

Qualifications

• Current Certified Nursing Assistant (CNA) certification required
• Valid driver’s license with a clean driving record
• Ability to safely assist residents with mobility and transfers
• Strong organizational and time-management skills
• Excellent communication and interpersonal skills
• Ability to work independently and adapt to changing schedules

Working Conditions

• Primarily day shift (7:30 AM – 4:00 PM)
• Flexibility required for early, late, or extended hours based on resident appointment needs
• Participation in holiday rotation required
• Frequent lifting, standing, and assisting residents with mobility
