Minimum Requirements
- Master’s Degree in Speech-Language Pathology from an accredited program.
- Completed Clinical Fellowship Year (CFY) and current Certificate of Clinical Competence (CCC) from ASHA preferred.
- Active New Jersey state license in Speech-Language Pathology required to start the assignment.
- Current CPR certification.
- Valid driver’s license, vehicle insurance, and reliable access to a vehicle or use of public transportation for home visits.
- Experience providing home health therapy or comparable community-based care preferred.
- Ability to perform evaluations, develop plans of care, and document per CMS and agency standards.
- Availability for schedule flexibility including participation in a weekend rotation and potential observed holiday availability as needed.

Job Summary and Responsibilities
Provide evaluation and skilled treatment for patients with speech, language, cognition, voice, resonance, and swallowing disorders in home health settings. Develop individualized plans of care in coordination with physicians and interdisciplinary team members. Complete timely documentation, progress notes, and discharge summaries per agency and CMS requirements. Educate patients, families, and caregivers. Participate in staff meetings, in-service education, and agency initiatives. Maintain required continuing education to support licensure and best practices.
