Position Summary
Richmond Health Care Center is seeking a compassionate and motivated Social Worker to join our team. The Social Worker plays a vital role in supporting residents and their families by addressing psychosocial needs, coordinating services, and assisting with care planning and discharge planning.
Key Responsibilities
Complete psychosocial assessments for residents upon admission and throughout their stay
Assist residents and families with adjustment to placement and changes in health status
Coordinate and facilitate discharge planning with the interdisciplinary team
Provide emotional support and counseling to residents and families
Participate in care plan meetings and interdisciplinary team meetings
Assist residents in obtaining community resources and support services
Advocate for resident rights and ensure residents maintain dignity and quality of life
Maintain accurate documentation in accordance with facility policy and regulatory requirements
Assist with resident grievances and ensure timely resolution
Support admissions by communicating with families and referral sources when needed
Qualifications
Bachelor’s Degree in Social Work (BSW) required; Master’s Degree (MSW) preferred
Current state licensure (if applicable)
Experience in long-term care, skilled nursing, or healthcare setting preferred
Strong communication, organizational, and problem-solving skills
Ability to work collaboratively within an interdisciplinary team
Knowledge of discharge planning and community resources
