Schedule: Days | Full-time | 8 am - 4:30 pm Mon - Fri

Salary: $46,800.00 - $68,445.00 per year

• Empower patient resilience by assessing social and emotional barriers and crafting personalized intervention plans that help patients navigate life's most difficult transitions.
• Build a bridge to recovery by consulting with clinical teams and external agencies, ensuring that the treatment plan is fully supported by a network of family and professional resources.
• Champion community connection by identifying and securing vital health and welfare resources, acting as a creative problem-solver to meet each client’s unique socioeconomic needs.
• Safeguard the patient environment by recognizing safety hazards or emotional crises and taking immediate action to ensure a secure, supportive setting for healing.

Licensure / Certification / Registration:

• Master Social Worker, preferred. 

Education:

• High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.

Work Experience:

• 3 years of experience preferred.

Long Term Care experience.
