A DAY IN THE LIFE OF A RESOURCE SPECIALIST

In this position, you will provide person-centered care coordination with older Minnesotans and caregivers over the phone and in-person. You will be providing options counseling, advocacy, resource location assistance and education. You will research home and community-based service optionsandassist clients as they navigate systemsand comply with specific protocols and documentation requirements. 

QUALIFICATIONS

2 years of experience with case management or care coordination in a long-term care,hospital,or community setting

  • Compliance with all state licensure requirements
  • Bachelor's degree from an accredited college or university in a human services-related field. Social work and nursing degrees strongly preferred.
  • Reliable transportation
  • Heart for working with the elderly

WORK SCHEDULE FOR RESOURCESPECIALIST

This position worksMonday - Friday, 8:00 AM - 4:30 PM, with two to three in-office days per week and theremaining time spent working from home.
