Job Summary:
Our client is seeking an experienced and compassionate Social Worker with Emergency Department (ED) experience to join their Care Management team. The successful candidate may be eligible for a $15,000 sign-on bonus to be paid within the first year of a two-year commitment.

- Collaborate within a Care Management Triad Team Model consisting of:
- RN Case Manager – Drives patient progression and discharge planning
- Social Worker – Addresses psychosocial barriers and supports discharge needs
- Utilization Management Specialist – Reviews patient status and payer coverage
- Assess psychosocial needs of patients and families in the Emergency Department
- Develop individualized care plans and coordinate discharge planning
- Advocate for patients and connect them to appropriate resources and support services
- Ensure compliance with regulatory requirements and hospital policies
- Master's degree in Social Work (MSW) or Master of Social Services (Client) from an accredited graduate school (Required)
- LCSW or LMSW (State of Delaware) required prior to start date
- Acute Care Social Work experience preferred
- Experience working with neonates, children, and families is required for Maternal-Child positions
- Certified Case Manager (CCM) preferred within 18 months of eligibility
