Department: Emergency Department

Schedule: Weekend Overnights | Thursday, Friday, and Saturday 7p-7a and Wednesday 7p-11p

How you'll make an impact in this role:
  • The social worker’s responsibilities include discharge planning, linkage to community resources, and coordinating placement in psychiatric facilities.
  • Respond to a variety of situations including, but not limited to, suicidal ideation and attempts, psychosis, substance abuse, domestic violence, sexual assault, and homelessness.
  • Duties include identifying and notifying next of kin for critical trauma patients and providing support for cases involving death.
  • Lead the design and execution of personalized care plans, ensuring every step from admission to discharge reflects the patient’s unique values and recovery goals.
  • Advocate for patients by identifying and removing social or financial barriers to care, ensuring they have the resources needed for long-term health.
  • Support families through complex psychosocial challenges, offering expert guidance during crises, substance abuse recovery, or difficult life adjustments.
  • Collaborate with the interdisciplinary care team to refine treatment strategies, ensuring seamless communication and high-quality medical documentation that drives better clinical outcomes.
  • This position collaborates with other members of the healthcare team, as well as other community agencies and providers.
What minimum qualifications you'll need:

Licensure / Certification / Registration:

  • One or more of the following required:
    • Clinical Counselor obtained prior to hire date or job transfer date.
    • Counselor credentialed from the Texas State Board of Examiners of Professional Counselors obtained prior to hire date or job transfer date.
    • Social Worker credentialed from the Texas State Board of Social Worker Examiners obtained prior to hire date or job transfer date.
    • Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
  • Case Manager credentialed from the American Case Management Association (ACMA) preferred.

Education:

  • Bachelor's degree in Social Work required.

Work Experience:

  • 3 years of experience preferred.
What additional requirements you'll need:
  • Active State Licensure: Current and unrestricted license in Social Work (LMSW/LCSW) as required by state regulations.
  • Clinical Case Management Experience: A proven track record of coordinating complex discharges and care transitions within an acute care or clinical setting.
