How you'll make an impact in this role:Champion patient-centered care by designing integrated care plans that align medical necessity with the unique values and preferences of patients and their families.
Navigate complex psychosocial barriers by assessing and treating needs related to crisis intervention, substance abuse, and legal challenges to ensure holistic recovery.
Advocate for essential resources by identifying and removing systemic or financial obstacles that prevent patients from accessing the care they deserve.
Orchestrate seamless transitions through constant collaboration with the care team, ensuring the health plan remains effective and responsive as the patient’s needs evolve.
Guide post-discharge success by providing expert recommendations on complex levels of care and documenting the clinical journey to ensure high-quality continuity of care.
Licensure / Certification / Registration:
One or more of the following required:
- Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
- Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred .
Education:
- High School diploma equivalency with 2 years of cumulative experience OR Associate's
degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
