Salary: $51,848.16 - $75,819.95 per year
How you'll make an impact in this role:- Conduct patient interviews, assess care priorities, and document all case activity to support accurate treatment planning.
- Collaborate with managers, care teams, external agencies, and patient families to ensure alignment with established treatment plans.
- Connect clients and families to appropriate community resources, supporting access to needed services and assistance.
- Maintain strong partnerships with community health and social service agencies while proactively identifying safety hazards and initiating corrective actions to protect residents.
Licensure / Certification / Registration:
- Master Social Worker, preferred.
Education:
- High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
Work Experience:
- 3 years of experience preferred.
Experienced Licensed Social Worker
1-2 years of experience in social services and case management
Proficient in assessments, discharge planning and care coordination
