Position Summary
The Inpatient Case Management Social Worker provides comprehensive psychosocial support and discharge planning services to hospitalized patients. This role supports acute care units by conducting assessments, coordinating care plans, and collaborating with interdisciplinary teams to ensure safe and effective transitions of care. The position serves a diverse adult patient population and requires prior acute care experience.
Key Responsibilities
Conduct biopsychosocial assessments and develop individualized care plans
Provide discharge planning and care coordination for inpatient populations
Deliver 1:1 patient support and facilitate group counseling as appropriate
Collaborate with physicians, nursing staff, and interdisciplinary teams
Document patient interactions, assessments, and plans of care in EPIC
Support patients with complex medical, mental health, and social needs
Advocate for patient resources and continuity of care post-discharge
Required Qualifications & Skills
Active AK LMSW or LCSW license (pending license accepted)
Minimum 1 year of experience in an acute care setting
BLS certification required
Strong documentation and care planning skills
Biopsychosocial assessment experience
EPIC EMR experience required
Preferred Qualifications
Experience working with medically complex or geriatric populations
Strong interdisciplinary collaboration skills
