Job Title: Social Care Network (SCN) Transportation Coordinator
Location: Clifton Springs

Job Summary:

In accordance with the organization’s mission, vision and values, the Social Care Network (SCN) Transportation Specialist is responsible for providing and coordinating transportation services that address health-related social needs (HRSNs), ensuring individuals can access medical, behavioral health, and community-based services. This role supports engagement and continuity of care by reducing transportation barriers, coordinating with care teams and community partners, and providing person-centered support during interactions. The Transportation Specialist also observes and communicates emerging needs, helping connect individuals to additional SCN services that promote stability, access, and improved health outcomes.

Minimum Qualifications:

High School Diploma or equivalent required. Relevant experience in human services, transportation, behavioral health, or working with underserved populations preferred. Ability to engage individuals in a person-centered manner and knowledge of community resources preferred. Experience supporting individuals with substance use, mental health, or chronic conditions preferred. CRPA certification preferred but not required; candidates with lived experience and willingness to pursue certification are encouraged to apply. Valid New York State Driver’s License with a safe driving record required.

Duties: 

1. Provide safe, reliable, and timely transportation to support access to social care services, including housing appointments, benefits offices, food resources, and other community-based supports.

2. Coordinate transportation requests with SCN team members and community partners to ensure individuals can access needed services and supports.

3. Engage individuals in a respectful, person-centered manner during transport, promoting a supportive and recovery-oriented environment.

4. Support outreach and engagement efforts by assisting individuals in connecting to services and maintaining involvement in care.

5. Conduct Health-Related Social Needs (HRSN) screenings, as appropriate, to identify unmet social needs such as housing instability, food insecurity, and transportation barriers, and communicate findings to the care team.

6. Observe and report any changes in an individual’s presentation or behavior, communicating concerns to appropriate staff.

7. Identify potential health-related social needs (HRSNs) and communicate these needs to the care team for follow-up and coordination.

8. Maintain accurate and timely documentation of transportation services, interactions, and any notable incidents in accordance with agency and SCN requirements.

9. Ensure compliance with all agency policies, including vehicle operation, safety procedures, and confidentiality standards.

10. Monitor vehicle condition, report maintenance needs, and ensure vehicles are clean and ready for use.

11. Participate in team meetings, trainings, and collaborative discussions to support coordinated service delivery.

12. Communicate effectively with supervisors, team members, and community partners to support service coordination.

13. Serve as part of the program team, ensuring communication across the team and adherence too and implementation of policy/procedures.

14. Provide supportive, prosocial interactions with person transporting that is Person-Centered and adheres to FLACRA Mission, Vision and Values 

15. Perform other duties and functions as directed. 

 

Other Skills/Knowledge and Experience

1. Excellent written and verbal communication skills. 

2. Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers.   

3. Working knowledge of behavioral health issues.  

4. Comfort/ability to intervene in crisis/support de-escalation of struggling clients

5. Ability to multi-task, have good problem solving & time management skills and the ability to remain calm in crisis. 

6. Ability to take constructive criticism as well as be a solution oriented leadership team member.

Personal Qualities:

Subscribes to person centered strength based philosophy of care, engages well with others, excellent communication skills, ability to work in a large team atmosphere.

Licensure/Certifications

New York State driver’s license, safe driving record and availability of personal vehicle for work.  Daily travel is required including the transport of clients.
