Senior LTSS Service Care Manager (RN) Highlights
• The pay for this position is $46.00-$49.00 per hour
Monday – Friday | 8:00 AM – 5:00 PM with possible overtime
Remote with field travel within San Antonio and surrounding counties
• This position is a 6-month contract with potential for extension or conversion

Responsibilities
• Conduct in-home assessments for pediatric members enrolled in the program to evaluate healthcare and service needs
• Identify potential unmet needs and recommend appropriate Medicaid services based on medical conditions and assessments
• Complete approximately 6 member visits per week , typically 2 visits per day across 3 days , with additional days dedicated to documentation
• Travel from home office to member homes within assigned coverage areas
• Complete detailed clinical documentation and submit reports within established turnaround times
• Communicate with internal care teams and leadership through remote collaboration tools
• Maintain compliance with documentation standards, training requirements, and organizational performance metrics
• Support care coordination initiatives to ensure members receive appropriate healthcare services and resources

Requirements
Active Registered Nurse (RN) license
ADN, BSN, or MSN in Nursing
3+ years of nursing experience
2+ years of case management experience
• Valid driver’s license and ability to travel for in-home member visits
• Strong organizational, time management, and communication skills
• Ability to manage documentation-heavy workloads and meet deadlines

Preferred Qualifications
• Home health experience
• Pediatric care experience
• Prior experience conducting in-home assessments or care coordination

  • Maintain radiation safety standards as outlined in the radiation safety manual

  • Perform ongoing job expectations in accordance with RiverMend Health's scientific model strategies and operating guidelines
  • Maintain working knowledge of industry standards related to renal replacement therapy in the hospital setting, including ESRD and AKI concepts/principles, regulatory, and accreditation standards
