The County of San Mateo Family Health Services Division is seeking an experienced and qualified Senior Community Worker - Spanish Speaking Preferred to serve in the Enhanced Care Management (ECM) Program within California Children's Services (CCS), Family Health Services Division. To be eligible for ECM, one must have CCS and have additional care management needs beyond the CCS condition. CCS is a tax-supported statewide program that helps children and young adults up to 21 years old who have certain medical conditions. CCS provides services such as arranging, directing, and paying for medical care, occupational and physical therapy, equipment, and rehabilitation. To learn more about the California Children's Services program, please visit https://www.smchealth.org/ccs
 
The Senior Community Worker will provide enhanced care management to the highest-need members and provide intensive coordination of health and health-related services to designated CCS clients through in-person field/home visits and electronic contacts via telephone or video calls. The Senior Community Worker will provide core services such as outreach and engagement, comprehensive assessment and care management planning, coordination of and referral to community and social support services, enhanced coordination or care, member and family support, health promotion, and comprehensive transitional care. Under the direction of the Community Program Supervisor, the Senior Community Worker will work in collaboration with the Medical Therapy Unit and Health Plan of San Mateo Whole Child Model to ensure client needs are met. Requirements of the position include knowledge and ability to easily navigate through electronic health records and provide accurate and descriptive documentation in client files.

The ideal candidate brings at least two years of recent experience conducting comprehensive assessments, care coordination, health advocacy, and referrals to community-based services. Familiarity with the CCS and ECM programs is highly desirable. This individual is able to work independently as well as collaboratively within a multidisciplinary team, communicates clearly and effectively both verbally and in writing, and demonstrates flexibility in adapting to changing environments. Possession of a valid California Driver’s License or equivalent is also required.
