RVNAhealth is seeking a self-motivated, compassionate RN to join our Nurse-Family Partnership (NFP) team. This evidence-based, community health program transforms the lives of parents and their children by providing vital home-based nursing support from pregnancy through the child's second birthday.
As a Nurse Home Visitor, you will work directly with eligible individuals and families, delivering nursing education in the home setting. You'll play a pivotal role in promoting healthier pregnancies, stronger parent-child relationships, early childhood development, and long-term family success.
Position Highlights:
- Visit clients in their homes
- Work remotely between visits
- Travel required within the Waterbury/Danbury region
- Flexible schedule
- Full-Time schedule of 37.5 hour per week
- Occasional evening or weekend visits required to meet client needs (scheduled in advance by nurse; flex time provided)
- Work with diverse populations impacted by economic and health disparities
- Spanish-speaking candidates highly preferred
- Education-based nursing position; minimal hands-on nursing care
- Extensive training and support are provided
Essential Responsibilities:
- Provide regular home visits to pregnant individuals and their families
- Educate clients on prenatal health, nutrition, infant care, child development, and goal setting
- Adhere to the NFP model and the nursing process
- Carry a caseload of up to 25 clients
- Participate in initial and ongoing NFP education
- Collaborate with interdisciplinary partners and community resources
Qualifications:
- Current RN license in good standing and CPR certification required at time of hire.
- Two years of recent experience in maternal/child health, public health, behavioral health, or home visiting preferred.
- Excellent written, verbal, and basic computer skills.
- Spanish language fluency highly preferred.
- Individual must be self-directed, enjoy flexibility, and be comfortable with variety and unpredictability in their daily work.
- Must have a valid driver's license and vehicle to travel throughout the specified area.
