Location: Thunderbolt, GA 31404. (Service area includes Effingham, Bryan, and Chatham Counties)

Pay: $50.00 per hour

Schedule: Monday-Friday (until shift completion). On-call rotation may be required based on staffing and branch needs

Expected Weekly Productivity: 30 units

Overview

We are seeking a compassionate and experienced Registered Nurse (RN) for Home Health services based out of Thunderbolt, GA. In this role, the RN will provide skilled nursing care to patients in their homes, develop and implement individualized care plans, and coordinate services with interdisciplinary care teams to ensure quality and continuity of care.

Key Responsibilities

• Perform comprehensive patient assessments in the home health setting.
• Develop, implement, and evaluate individualized Plans of Care (POC) .
• Provide skilled nursing care including wound care, medication administration, IV therapy, and patient monitoring.
• Educate patients and families on medications, disease management, home safety, and self-care practices.
• Maintain accurate clinical documentation and patient records.
• Coordinate care with physicians, interdisciplinary team members, and community resources.
• Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
• Supervise home health aides and provide clinical direction as appropriate.

Required Qualifications

• Current Registered Nurse (RN) license – required
• CPR certification – required
• Homecare Homebase experience – required
• OASIS experience – required
• 1–2 years of clinical RN experience – required
• 2+ years of home health experience – required

Preferred Qualifications

• CHPN certification – preferred
• Bachelor of Science in Nursing (BSN) – preferred
