RN FIELD CASE MANAGER (HOME HEALTH) | DETAILS AND COMPENSATION:

• Location: Fond du Lac WI 54935 – 100% on-site, home health settings
• • Field Work: On-site visits in Fond du Lac, Dodge, and Columbia counties.
• Payrate: $56.14/hr
• Required Availability: Full-Time | Monday – Friday, 8:00 AM – 4:30 PM (No on-call or weekends)
• • Caseload: Approximately 5 visits per day.

RN FIELD CASE MANAGER (HOME HEALTH) | SUMMARY AND HIGHLIGHTS:

The RN Field Case Manager will provide comprehensive clinical assessments and personalized care coordination for high-risk patients within home health and hospice settings. This role involves managing daily patient visits, delivering skilled nursing care such as wound and pain management, and serving as a primary advocate for patients and their families. Additionally, the Case Manager collaborates with a multidisciplinary team to ensure seamless care transitions and accurate documentation of health outcomes.

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs

RN FIELD CASE MANAGER (HOME HEALTH) | RESPONSIBILITIES:

• Patient Assessment: Perform comprehensive assessments of high-risk members and identify opportunities for health promotion and illness prevention.
• Care Coordination: Collaborate with primary care providers, social workers, chaplains, and families to implement individualized care plans.
• Clinical Management: Provide wound care, pain management, and education on end-of-life signs and symptoms.
• Transition Management: Coordinate care transitions and act as a patient advocate while ensuring strict HIPAA compliance and privacy.
• Documentation: Maintain detailed clinical and functional records using Electronic Medical Records (HCHB or MatrixCare).

RN FIELD CASE MANAGER (HOME HEALTH) | REQUIREMENTS:

• Education: Associate’s or Bachelor’s Degree in Nursing.
• Attendance: Commitment to mandatory attendance during the first 90 days.
• License: Current, unrestricted RN License for the state of Wisconsin.
• Certifications: Active CPR/BLS certification.
• Previous experience in Wound Care is required.
• Valid driver’s license, reliable vehicle, and active car insurance.

Preferred Qualifications

• Experience with HomeCare HomeBase (HCHB) or MatrixCare EMR systems.
• Prior experience in Case Management or Hospice care.
• Background working with frail elderly patients in home settings or skilled nursing facilities.
