RN CASE MANAGER - HOME HEALTH (FIELD WORK) | DETAILS AND COMPENSATION:
- Location: Westminster MD 21157 – 100% On-site, Field Work
- Coverage Area: Howard County
- Payrate: $56.14/hr + mileage
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
RN CASE MANAGER - HOME HEALTH (FIELD WORK) | SUMMARY AND HIGHLIGHTS:
The RN Case Manager is responsible for coordinating and managing care for high-risk members to ensure safe, effective, and cost-efficient healthcare outcomes. This role involves comprehensive patient assessments, care coordination across disciplines, and strong patient advocacy in a home health setting.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
RN CASE MANAGER - HOME HEALTH (FIELD WORK) | RESPONSIBILITIES:
- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to develop and implement individualized care plans
- Continuously evaluate and adjust appropriate levels of care
- Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
- Arrange non-medical support services (e.g., housing, transportation) to support treatment compliance
- Engage specialty and community resources as needed
- Maintain detailed documentation of clinical, functional, and fiscal outcomes
- Promote health education, illness prevention, and early intervention
- Act as a patient advocate while maintaining privacy and confidentiality
RN CASE MANAGER - HOME HEALTH (FIELD WORK) | REQUIREMENTS:
- Education: Associate’s or Bachelor’s Degree in Nursing or related field
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active, unrestricted Maryland RN license (compact license accepted)
- 2+ years of home health experience highly preferred
- Case Management certification
- Strong ability to analyze complex medical information and make sound clinical decisions
Preferred Qualifications:
- Experience with HomeCare HomeBase (HCHB) and OASIS
- Clinical skills including: Trach care, PICC lines, Wound vac and wound care, IV therapy, Drain care, Phlebotomy, PEG tubes, PleurX drains, Ostomy care
