RN Case Manager (Contract to Hire)
Location: Owensboro, KY 42303 (On-site / Field-based)
Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM
Pay Rate: $56.16/hour
Work Type: Hybrid (primarily field-based)
Contract Type: Contract to Hire
About the Role
A-Line Staffing is seeking a motivated and detail-oriented RN Case Manager to support high-risk members through comprehensive assessments, care coordination, and individualized care planning. This role focuses on improving patient outcomes through collaboration, advocacy, and efficient care management.
This is a contract position with strong potential for permanent hire based on performance, attendance, and business needs.
Primary Responsibilities
- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized, comprehensive care plans
- Identify, address, and continuously reassess cost-effective and appropriate levels of care
- Coordinate care transitions across the continuum of care
- Arrange non-medical support services to support treatment compliance (e.g., housing, transportation)
- Partner with physicians, social workers, discharge planners, and claims professionals
- Engage specialty resources to support optimal patient outcomes
- Maintain detailed documentation of clinical, functional, and fiscal outcomes
- Identify opportunities for health promotion and illness prevention
- Prevent adverse patient occurrences when possible and intervene promptly when prevention is not feasible
- Act as a patient advocate while ensuring privacy and confidentiality
Qualifications
- Associate’s or Bachelor’s Degree in Nursing or related field
- Active, unrestricted Registered Nurse (RN) license in the state of practice
- Strong clinical judgment with the ability to analyze complex medical information
- Case Management certification preferred
- Commitment to mandatory attendance during the first 90 days
