To support our continued growth in Lincoln, the first four to six months of this role will be paid on a salary basis as we build our Lincoln census. During this time, additional responsibilities will include assisting with marketing efforts in Lincoln, helping cover patient visits in Omaha as needed, and assisting with planning educational materials. This is a unique opportunity to help shape and grow our presence in the Lincoln market while playing a vital role in expanding access to exceptional care.

Job Summary
Join Aegis as an RN Case Manager where you will play a crucial role in delivering exceptional care to our patients. Your compassionate approach will ensure comprehensive physical, social, and psychological assessments, and your friendly and supportive demeanor will actively engage patients, families, and the rest of our Interdisciplinary TEAM in developing comprehensive care strategies and plans. With your attention to detail, you will efficiently coordinate patient care delivery, manage resources, and maintain meticulous patient records. Your positive collaboration with other TEAM members, patients, and their families, along with proactive initiation of referrals, will create a holistic care experience.

Desired Skills & Qualifications
Current Registered Nurse (RN) license in Nebraska
Home Health/Hospice experience as a Registered Nurse preferred
Ability to utilize full range of movement to transfer patients
Effective verbal & written communication and time management skills
Valid Driver's License (clean record) and reliable transportation
Proof of current auto insurance
Current CPR Certification
Negative TB Test
