Location: Tallahassee, FL
Shift: Evening Shift (12:00 PM – 8:30 PM EST)
Schedule: 1 weekend per 4-week rotation
Employment Type: Full-Time

Compensation & Incentives

Base Pay: $31.94 – $43.92 per hour (based on experience)

Shift Differentials:

• Evening Shift: +$2.50/hr

• Weekend Shift: +$2.00/hr

• Evening + Weekend: +$4.50/hr (stackable)

Sign-On Bonus: $10,000 (paid in five $2,000 installments over a 2-year commitment)

Position Overview

The RN Case Manager (Care Coordinator) is responsible for facilitating and coordinating the interdisciplinary plan of care. This role focuses on evaluating clinical appropriateness, medical necessity, and level of care while ensuring efficient resource utilization. The Case Manager plays a vital role in promoting positive patient outcomes, supporting patient throughput, identifying barriers to care, and coordinating safe and effective discharge planning.

Key Responsibilities

Assessment & Care Planning

• Conduct comprehensive medical and psychosocial assessments

• Develop, implement, and document individualized case management plans

Interdisciplinary Leadership

• Lead and participate in interdisciplinary care team meetings

• Collaborate across departments to coordinate and evaluate patient care

Utilization Management

• Review admissions for medical necessity using approved criteria

• Conduct concurrent and retrospective reviews for third-party payers

Throughput & Discharge Coordination

• Identify and resolve barriers impacting patient flow

• Facilitate timely and appropriate discharge planning

• Coordinate post-discharge services to ensure continuity of care

Communication & Liaison

• Serve as a liaison between physicians, patients, families, hospital staff, and external agencies

Qualifications

Experience:

• Minimum 3 years of recent (within the last year) Case Manager experience in an acute care setting

OR

• 3 years of experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED

Home Health/Insurance Background:

• Must have 3 years of total acute care experience, with at least 1 year within the past 5 years

Education:

• Associate Degree in Nursing (ADN) or Diploma in Nursing required

• Bachelor of Science in Nursing (BSN) preferred

Licensure:

• Active Florida RN license or Compact RN license

Certifications (Preferred):

• Case Management

• Nursing Specialty Certification

• Utilization Review
