Job Summary and Qualifications
Schedule: 7am-5:30pm (10 hours) 2 days a week
Provides direct and/or indirect professional nursing care to achieve the goals of the Meadows cardiopulmonary Rehabilitation programs. The primary goals are the achievement and maintenance of optimal physical and psychological wellbeing through positive lifestyle changes of individual with heart disease or pulmonary disease. Responsible for planning, organizing, and evaluating the quality of care rendered by Meadows personnel. Understands and demonstrates operation excellence skills in performance of job duties.
What you will do in this role:- Manages the care of each patient receiving oximetry, telemetry, blood pressure monitoring or others as needed.
- Applies principles and policies or participant selection, exclusion criteria and case management in the care of the Cardiac Rehab patients.
- Competently teaches various classes related to cardiac and pulmonary disease. Provides day to day interaction with clients as well as case management for their clinical care. Develops and instructs patients on appropriate home exercise programs and provides follow up for retention program.
- Obtains appropriate informed consent physician referral, and exercise prescription documentation while also conducting quality of life assessments and risk stratification/disease severity in a timely fashion.
- Demonstrates competency with clinical computer system/record keeping, progress reports, Dynamap, glucose monitoring, and documentation.
- Checks emergency supplies and equipment for proper function and makes sure there are ample supplies for work. Sets up and supervises emergency drills and mock codes for staff.
- Demonstrates competency in the interpretation/intervention of dysrhythmias while appropriately terminating outpatient exercise programs.
