This Is Nursing at Its Most Meaningful.
At Riverside Home Health & Hospice, we have been serving patients and families in Southern Oregon for over 20 years. In the last several years, we have expanded our commitment to include Hospice care - providing comfort, dignity, and support during life's most sacred moments.
We are seeking a full-time Hospice Registered Nurse who understands that this work is more than clinical skill - it is presence, leadership, and compassion in action.
Who This Role Is Designed For
This role is ideal for a nurse who:
• Finds purpose in end-of-life care
• Is confident performing comprehensive patient assessments
• Thinks holistically - physically, emotionally, and spiritually
• Leads calmly and clearly in sensitive situations
• Takes ownership of patient outcomes and family support
• Is self-directed and comfortable working independently
Hospice nursing requires both strong clinical judgment and a steady heart. If that describes you, this may be your calling.
What Makes Hospice at Riverside Different
• One-on-one care in the patient's home
• Deep, relationship-driven support for families
• Strong interdisciplinary collaboration
• Opportunity to supervise and mentor hospice aides
• Involvement in quality improvement initiatives
• A leadership culture that develops people - not just positions
As part of Pennant Group, we are a leadership-focused healthcare organization that believes in elevating clinicians who lead through their work.
What You'll Be Doing (High Level)
• Completing comprehensive initial and ongoing hospice assessments
• Developing, implementing, and revising individualized plans of care
• Managing symptoms and providing palliative nursing interventions
• Administering medications and treatments per physician orders
• Counseling and educating patients and families
• Coordinating services and ensuring necessary equipment/resources
• Supervising hospice aides and collaborating with ancillary personnel
• Participating in on-call rotation as defined by policy
• Contributing to quality assessment and performance improvement initiatives
This role carries responsibility - and real impact.
Our Culture: Leadership at Every Level
We operate by our CAPLICO Core Values:
• Customer Second (Employee First)
• Accountability
• Passion for Learning
• Love One Another
• Intelligent Risk Taking
• Celebration
• Ownership
Leadership is not about title. It's about how you show up - for patients, families, and teammates.
What You'll Need
• Graduate of an accredited school of nursing
• Active RN license in Oregon
• Current CPR certification
• Minimum two years nursing experience (hospice, home care, or public health preferred)
• Strong assessment and communication skills
• Ability to organize and prioritize independently
• Valid driver's license and insured vehicle
Hospice care requires mobility, lifting, and active patient engagement in home settings.
