Overview

Chestnut Health Systems is seeking a Clinician II (Residential Service Coordinator) to provide recovery-oriented, trauma-informed mental health services to individuals involved in housing programs. This role supports consumers in achieving stability, wellness, and independence through individualized, consumer-driven services delivered in both office and community-based settings.

 

The Clinician II plays a critical role in assessments, treatment planning, service delivery, and coordination with community and internal partners, particularly those supporting housing stability and substance use recovery.

This full-time position will work Tuesday through Saturday from 3:00pm to 11:30pm or Tuesday through Saturday from 7:00am to 3:30pm. 

Responsibilities

Clinical Services & Documentation• Complete all required data entry and paperwork in accordance with program and agency policies, including assessments, treatment/recovery plans, progress notes, and funder requirements• Ensure 100% of assessments and treatment/recovery plans are completed prior to due dates• Submit at least 80% of progress notes and required documentation within 48 hours of service delivery

 

Direct Consumer Care• Provide services outlined in individualized treatment/recovery/safety plans that are consumer-driven and program-compliant• Maintain assigned productivity expectations• Complete service pre-authorizations and re-authorizations accurately and timely• Conduct screenings for new housing referrals when assigned

 

Recovery-Oriented Practice• Promote the recovery model of care, including trauma-informed approaches, hope, respect, empowerment, wellness, and connectedness• Utilize recovery-oriented language in all documentation and interactions

 

Coordination & Advocacy• Provide referrals, linkage, and advocacy to community resources, with emphasis on substance use treatment and housing-related supports• Collaborate with internal programs, particularly substance use services, and engage natural support systems to promote positive outcomes

 

Team Participation & Leadership• Attend meetings and consumer staffings and participate in program development activities• Provide backup and support to team members as needed

• Attend and document 100% of required Rule 132 supervision if not a LPHA• Complete all mandatory Chestnut trainings and CEU requirements, when applicable• Assume leadership responsibilities as assigned

 

Evidence-Based & Community-Based Services• Utilize evidence-based practices in individual, family, and group therapy• Provide services in consumers’ natural environments and community settings when appropriate

 

Professional Standards• Maintain strict confidentiality of all consumer and organizational information• Perform additional duties as assigned or negotiated with the supervisor

Qualifications

 

• Bachelor’s degree with 5 years of supervised mental health experienceOR• Master’s degree in counseling, psychology, social work, or a related field with less than 1,000 hours of supervised mental health experience• Valid driver’s license, private automobile insurance, and insurability• Basic computer proficiency, including MS Word, Excel, Outlook, Adobe Acrobat, and electronic medical records (EMR)
