RESPONSIBILITIES OF THE JOB AND ESSENTIAL ELEMENTS:
- Participates in treatment planning and case conferences with other staff of the community support program and with other agencies.
- Completes all assigned tasks from the daily schedule which may include: maintaining a therapeutic alliance with clients referring and linking clients to all needed services provided outside the agency and following up to see that said services are delivered.
- Coordinating the provision of medical and vocational services.
- Coordinating overall independent living assistance services and working with community agencies to develop needed resources including housing, employment options, and income assistance.
- Providing support and consultation with consumer’s family.
- Providing in-home services including ADL training, medication assistance and training, social support and home management assistance.
- Providing supportive counseling.
- Documents and keeps other staff, clinical coordinators, and Program Director informed of all changes in the consumer’s mental or social status.
- Carries primary or secondary cell phone as assigned and responds to after hour and emergency calls in accordance with program directives.
- Provides education and works as an advocate for persons served in the community.
- Performs other duties as requested or assigned, verbally or in writing.
MINIMUM EDUCATION AND EXPERIENCE QUALIFICATION REQUIREMENTS:
- High School Diploma.
- At least two years of experience supporting individuals with severe and persistent mental illness.
REQUIRED SKILLS:
- CPR/First Aid Certification.
- Valid Driver’s License with clean driving record.
- Satisfactory Criminal Background Investigative Report.
- Crisis Prevention and Intervention Training.
- LLAM Certification.
- Training in Defensive Driving.
- Training in Infection Control.
- Training in Suicide Risk Management.
- Training in Working with Persons with Serious Mental Illness.
- Training in PM-46 and how to identify and report abuse, neglect, mistreatment, and misappropriation of property.
- Training in 45 and 42 CFR, Rules and Regulations that safeguard a person’s privacy.
- Training about Agency policies and procedures on both a daily and emergency basis.
- Training in Fire Safety and Evacuation.
- Training in Safe Food Handling.
- Training about local resources in the community.
NETWORK FUNCTIONS:
- Maintain and abides by OSHA requirements and guidelines.
- Observes all Fire and Safety Policies and Procedures.
- Observes all Personnel Policies and Procedures.
- Observes all Agency’s Policies and Procedures.
- Observes all NJDOH-DMHAS, CARF and NJSAMS Regulations.
- Observes all HIPAA Policies and Procedures and maintains confidentiality.
- Completes and returns all paperwork distributed by the Human Resource Department.
- Attends all required and scheduled trainings and meetings.
- Always maintain a professional demeanor and exercise good professional judgment in all areas of employment duties.
- Documents time worked accurately
EQUIPMENT/MACHINES:
- Telephone, cellular telephone, copier, fax, computer, calculator.
COMPUTER SOFTWARE APPLICATIONS:
- Computer literacy in MS Office products; Word, Excel, Outlook, Power Point.
- Agency Electronic Medical Record and other software programs.
PHYSICAL DEMANDS:
Individual must be able to meet physical requirements of the job:
- Sitting at a meeting table or desk - Reaching - Lifting - Bending - Standing - Stooping - Twisting - Climbing steps - Driving a vehicle.

Blood borne Pathogens, Right- To- Know, HIPAA, Security Acts, Fire & Safety.
