Job Title: Registered Nurse (RN) - Case Manager
Location: Summit, NJ
Duration: 2 Months+ contract - Onsite Role

Shift: 8 am – 4 pm/ No weekends/no on-call.

Pay Rate: $57/HOUR. on W2

PER DIEM RATES PROVIDED TO THE CANDIDATE LIVING MORE THAN 50 MILES FROM THE FACILITY.

Duties:

• Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process.
• Performs utilization management and quality screenings.
• In- Patient Case Management is required.
• Discharge planning exp required

Requirements:

• 2 years of acute care experience strongly preferred
• Discharge planning, In-Patient Case Management is required
• BSN Preferred / ASN - Required.
• RN NJ state License or multi state
