Job Details
What You Will Be Doing
Conduct home care admissions, reassessments, and patient visits based on physician orders
Coordinate and supervise HHAs to ensure consistent, high‑quality care
Initiate and update the plan of care, communicating changes in condition to the physician
Provide patient & caregiver education on disease processes, medications, and treatment plans
Document assessments, interventions, teaching, and progress in compliance with regulations
Maintain a manageable Monday–Friday caseload during regular business hours
Skills You Need
Active RN license (NYS)
Bilingual ability (English + Spanish, Creole, Cantonese, or Mandarin)
Strong communication and time‑management skills
Ability to work positively with diverse families and patient populations
Comfortable traveling to multiple homes in a single day
Fully vaccinated for COVID‑19 (required)
