POSITION: Registered Nurse Home Visitor The Nurse Home Visitor (NHV) is responsible for providing comprehensive nursing services to women and their families eligible for the Program. DUTIES: * Establishes and maintains therapeutic relationships while delivering individualized client case management services. *Conducts home visits to women and their families eligible for the NFP Program to determine patient and family needs * Adheres to nursing process and the NFP model of home visitation. * Performs home visits, including management of a caseload of up to 25 to 30 clients for a full time NHV. * Please see employers website for full job description. REQUIREMENTS: * A Bachelor of Science degree in Nursing. * Must be licensed in Colorado in good standing. * Current CPR. * A minimum of two (2) years related experience, knowledge of the Colorado State Nurse Practice Act. * Must possess and maintain a valid Colorado Driver's License and satisfactory driving record. * Must complete training within one year of receiving position. * Must be able to pass a background check and drug screening. PREFERRED SKILLS: * A minimum of two (2) years related experience in public, maternal child health and/or home health or visitation. * Spanish bilingual WAGE: $32.62 - $38.37 Hourly (Depending on Experience) BENEFITS: Medical, Dental, Vision, Retirement, Employee Assistance Program, Paid Time Off, and Retirement options.
