Job Details
What You Will Be Doing
Lead, guide, and supervise a multidisciplinary clinical team (Nursing, PT, OT, SLP, MSW, HHAs) to ensure exceptional patient care.
Oversee care delivery quality, compliance, documentation accuracy, and adherence to State and Medicare standards.
Participate in care conferences, staff instruction, team leadership, and training to maintain clinical excellence.
Ensure seamless coordination of home care nursing services with internal and external providers.
Support staffing decisions, recruitment efforts, on‑call coverage rotation, and occasional supervisory visits in Queens and NYC.
Skills You Need
Active RN license in New York.
OASIS and CHHA experience required.
3+ years of clinical experience with at least 1 year in home health/community health.
Strong assessment, communication, and clinical leadership skills.
Ability to collaborate effectively within interdisciplinary teams.
Valid driver’s license and insured vehicle.
