Job Details

What You Will Be Doing
Delivering high-quality nursing care in home health settings using the nursing process
Assessing patient needs and collaborating on individualized care plans
Educating patients and families using population-relevant materials
Evaluating care outcomes and adjusting plans as needed
Coordinating with interdisciplinary teams to ensure continuity of care
Participating in performance improvement and cost-effective care initiatives
Staying current with research and applying evidence-based practices

Skills You Need
Active RN license in North Carolina
At least 1 year of experience within inpatient is highly preferred
Strong clinical judgment and ability to manage complex patient needs
Excellent communication and collaboration skills
