Schedule: PRN, 12 hour Rotating shifts

How you'll make an impact in this role:
  • Architect comprehensive care plans that bridge the gap between admission and discharge, ensuring patient and family values remain at the heart of every clinical decision.

  • Evaluate patient progress and medical necessity with a critical eye to ensure every individual receives the precise level of care required for optimal recovery.

  • Advocate fiercely for your patients by identifying and removing systemic barriers, securing the essential resources needed for a seamless transition home or to the next phase of care.

  • Collaborate with interdisciplinary teams and leadership to refine health plans in real-time, maintaining a constant dialogue that guarantees no detail of the patient journey is overlooked.

  • Empower the clinical team by serving as a subject matter expert on medical documentation and discharge pathways, ensuring the quality of our records matches the quality of our care.

What minimum qualifications you'll need:

Licensure / Certification / Registration:

  • Licensed Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
What additional requirements you'll need:

Prior Acute Rehab or Case Management experience preferred.
