The Aortic Center Clinic (ACC) is a multidisciplinary program comprised of physicians, advanced practice providers, and nursing staff dedicated to the comprehensive care of patients with complex aortic disease.

Patients referred to the ACC require coordinated diagnostic testing and multiple specialty consultations to ensure accurate diagnosis and optimal treatment planning.

The Nursing Program Coordinator, in close collaboration with the ACC interdisciplinary team, plays a key role in coordinating and facilitating the clinical care of patients referred for ACC services. This role ensures seamless care delivery across the continuum, supporting timely evaluations, testing, and follow-up while serving as a central point of communication for patients and providers.

Responsibilities:

The Nursing Program Coordinator works with physicians and advanced practice providers to coordinate the care for patients who are referred for and receiving care for Aortic Center Program services. The Nursing Program Coordinator provides leadership by serving as the patient’s educator, consultant, and liaison for the medical, surgical, and nursing staff. The Nursing Program Coordinator participates in the coordination of patient care activities in collaboration with the multi-disciplinary health team, identifies and evaluates current clinical practices through performance improvement, and provides education when appropriate. The coordinator will manage the ACC database and identify care provision opportunities to integrate into clinical workflows.

 

60% Patient Care - Responsible for collaborating with physicians and APPs to increase care coordination between providers, patients, & administrative staff for acute & long-term follow-up care

 

  • Responsible for inputting testing orders and patient education for surgical candidates
  • Review of CAT scans & other imaging tests
  • Has direct accountability to Director of Service Line and Network Operations
  • Partners with ACC care team in the development patient education material & Aortic clinic informational sessions
  • Builds and maintains warm relationships with key stakeholders of care team
  • Ensures successful implementation and operations of new clinic workflows delivered in conjunction with Ross Ambulatory Care Center leadership
  • Oversee post-visit activities such as follow-up phone calls & scheduling of additional testing or physician visits
  • Ensures compliance with clinical best practices
  • Oversee the tracking and regular reporting of performance of ACC as a whole
  • Participates in regular meetings and other communication mechanisms with Ross ACC leadership
  • Partners with OSUWMC Regional Outreach Coordinators and other OSUWMC Service Line Network teams to ensure alignment and complimentary service offerings of program in relation to the broader enterprise
  • Partners with OSUWMC Marketing team(s) dedicated to HVC in developing and executing initiatives to internally and externally promote services, programs and brand awareness.

 

 

40% Program Coordination  

 

Screening & Referral TriageResponsible for reviewing patient chart and determining pre-visit needs for quality patient experience & streamlined operations

 

  • Responsible for identifying appropriate imaging test required for physician visit
  • Review of pre-visit needs to determine which specialties need to be involved in patient’s physician visit
  • Serves as a liaison for ACC for all services and programs outside of RHH, including, but not limited to:
    • Ambulatory clinical practices
    • Ambulatory ancillary clinical programs (advanced cardiac imaging, wellness and prevention services, etc.)
    • Academic initiatives including clinical education or lay-education events or programs
    • Physician relationships, recruitment, and support
    • Subspecialty program development
    • Community engagement initiatives
  • Supports HVC faculty by participating in Section, Division or other faculty meetings.  Serves as liaison for faculty and HVC/OSUWMC leadership in order to appropriately align goals, needs, strategy, etc.

 

 

 

Database Management:

  • Assistance with Aortic Center Registry database
  • Inputs patient pre-clinical information associated with Aortic Center visits into registry database
  • Works with Aortic Center APP to implement quality measures into clinical workflows
Qualifications:

 

  • For Hire: RN Nurse Education
    • Baccalaureate degree in nursing required from  aCCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program. 
  • On-going:
    • Minimum of 0-2 years experience in the clinical service area of cardiology, cardiac surgery or vascular surgery
    • Minimum 5 years preferred
    • Demonstration of TQM, leadership, communication, organizational and management skills.
    • Demonstrated experience in providing educational seminars
    • Current licensure of a Registered Nurse in State of Ohio
    • Current ACLS certification.
