Recovery Housing Case Manager

Vital Health provides integrated treatment for primary care, behavioral health, and substance use disorders for adults and children in Northwest Ohio. Our recovery housing program offers a safe, structured, and recovery-focused living environment for individuals working toward long-term sobriety.

We are seeking a Recovery Housing Case Manager with an active CDCA who understands Ohio Recovery Housing (ORH) standards and is passionate about supporting individuals in early recovery.

What You'll Do

• Complete intake assessments and develop individualized recovery housing plans
• Meet weekly with residents to monitor progress and support recovery goals
• Ensure compliance with Ohio Recovery Housing (ORH) standards and house policies
• Monitor sobriety expectations and address relapse warning signs
• Provide crisis intervention and coordinate higher levels of care when needed
• Assist residents with employment, benefits, transportation, and community resources
• Maintain accurate documentation and incident reports
• Collaborate with therapists, MAT providers, probation officers, and referral sources
• Support a safe, structured, and recovery-oriented living environment

Requirements

• Active CDCA (required)
• High School Diploma or GED (Associate's or Bachelor's degree preferred)
• 1+ year experience in addiction treatment, recovery housing, or behavioral health preferred
• Knowledge of substance use disorders and relapse prevention
• Familiarity with ORH standards strongly preferred
• Valid driver's license and reliable transportation
• Ability to work flexible hours as needed (some evenings/weekends)

Job Type: Full-time

Pay: $18.00 - $24.00 per hour
