Position Details:

Hartford HealthCare is seeking a Primary Care Nurse Practitioner or Physician Assistant to join our team! Our centrally located Plainville Primary Care practice offers a strong, stable team dynamic and a supportive work environment, led by highly engaged providers who are passionate about delivering quality care to the community. The Primary Care Provider will carry their own patient panel and provide quality, patient-centered care among a collaborative team.

Role: Open to Nurse Practitioners and Physician Assistants

Location: Plainville, CT

Setting: 100% Outpatient Primary Care – Adult Medicine (18+)

Schedule: Full-time, weekdays only, 4 day work week.

Call schedule: No call required

Nurse Practitioners and Physician Assistants Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.


Qualifications


Requirements:

·        Licensure: Unrestricted State of Connecticut Medical License (or eligible)

·        Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA

·        Experience: New graduates with strong internal medicine/primary care backgrounds are welcome to apply!

o   Preferred: 2+ years’ experience in a primary care or relevant outpatient setting
