Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current Oklahoma Physical Therapy license required prior to the start of assignment.
• Current CPR certification required.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle for travel within the agency service area.
• Experience providing home health physical therapy preferred.
• Ability to complete evaluations, develop plans of care, and document per CMS and agency requirements within state timelines.
• Willingness to participate in weekend rotation and potential availability on observed holidays as needed.
• Must be able to visit patients across the agency’s service area and float to other nearby agency locations as needed.
