Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky Physical Therapist license or ability to obtain prior to the assignment start.
• Current BLS (Basic Life Support) certification from AHA or ARC, or ability to obtain prior to start.
• Experience providing home health physical therapy, including patient evaluation, plan of care development, and skilled interventions in patient homes.
• Reliable transportation and ability to travel within assigned coverage areas, including Todd County and Logan County.
• Ability to perform documentation in electronic medical records and communicate with interdisciplinary teams.
• Preferred: Previous travel PT experience and familiarity with home health billing and regulatory requirements.
