Minimum Requirements
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification from AHA or ARC is Required
- 2+ years of acute nursing care (hospital base) and home care experience is Required
- Certified home health experience including proficiency with OASIS is Required
- Valid Driver License is Required

- Facility Type: Hospital
- Shift: Days
- Working Hours: 8 hours per day
- Weekend Requirement: Rotating weekends and holiday coverage required if needed
