Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active West Virginia state PT license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in skilled nursing facility settings preferred.
• Ability to provide documentation of professional credentials, immunizations, and background checks as required by facility.

Assignment Details

• Facility Type: Skilled Nursing Facility.
• Shift Name: Days.
• Shift Details: Monday–Friday, 0800–1630.
• Schedule: Five 8-hour shifts per week, 36 guaranteed weekly hours per client notes.
• Assignment Length: 13-week contract.
• Split position between local sites; candidate may be required to float between facilities within the region.
• Meal break: 30 minutes.
