Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active West Virginia state PT license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience in skilled nursing facility settings preferred.
- Ability to provide documentation of professional credentials, immunizations, and background checks as required by facility.
Assignment Details
- Facility Type: Skilled Nursing Facility.
- Shift Name: Days.
- Shift Details: Monday–Friday, 0800–1630.
- Schedule: Five 8-hour shifts per week, 36 guaranteed weekly hours per client notes.
- Assignment Length: 13-week contract.
- Split position between local sites; candidate may be required to float between facilities within the region.
- Meal break: 30 minutes.
